In my voluntary role as a COÖP leader, I will participate in the COÖP trip — hiking, biking, or river canoeing — from August 21 to August 24, 2019.

In consideration of my participation in the events, I knowingly:

• acknowledge and understand that my participation in the COÖP trip is entirely voluntary;
• understand that, although Columbia will take steps to foster COÖP trip safety, there are inherent risks in many activities and there are significant levels of personal responsibility that I must assume for myself as Columbia cannot guarantee my safety;
• agree to follow all the safety procedures, protocols, and instructions of any professionals who provide guidance and received through the training in the spring planning trip (including, but not limited to, Wilderness First Aid, Tri-State First Aid, Columbia staff member, COÖP coordinators);
• agree that I will participate in the activities as defined for the COÖP trip and not veer off independently from the activities scheduled for this trip;
• adhere to the alcohol free and drug free conduct defined for the COÖP trip;
• acknowledge that the Columbia administrative staff or agents attending the COÖP trip are not necessarily medically trained to care for any physical or medical problems of individuals participating;
• represent that I have adequate health and hospitalization insurance for any injuries that I may receive as a result of my participation in the events, and I understand that neither the University nor any of its agents or instructors serves as guardians or insurers of my safety;
• agree to use good personal judgment and exercise caution in any potentially high-risk situations at the site of the events; and
• recognize that I will continue to be governed by all applicable University policies and rules, and I agree to conduct myself in a professional manner at all times. If my conduct violates any policy or rule of the University, I understand that I may be required to leave the COÖP trip at my own expense, at the sole discretion of the University.
