The Office of the Dean will be ordering new polo shirts and name badges for its staff for use at official events, such as Class Day and Family Weekend. Everyone will receive a new Columbia College name badge, even if they already have one.

Please use this webform to indicate your shirt size and the name that you prefer to have printed on your new name badge by Friday, March 22.

If you do not respond with your preferences by then, we will order a name badge with your official name as is recorded in the People @ Columbia (PAC) system.
