I am exercising my free choice to participate voluntarily in the Columbia Outdoor Orientation Program’s (COOP) Fall trip. I recognize that this decision involves certain risks and I am willing to accept responsibility for those risks.
I understand that the goals of COOP are to provide a positive experience in a supportive group environment. I am aware that as a leader I am required to follow the policies and guidelines set by COOP and instructions from the Outdoor Orientation staff and coordinators. As part of Columbia University, COOP adheres to the University standards of respect for others. As a leader, I will facilitate a safe, positive, and supportive group dynamic for every first year student.
COOP is a substance-free program- alcohol or drugs of any kind are prohibited. Any violation of the substance policy may result in judiciary action.
I understand that I am solely responsible for any and all costs arising out of any bodily injury or property damage sustained during my participation in normal or unusual acts associated with the COOP Fall trip.
I hereby give my consent to Columbia University and thereby to leaders in the Columbia Outdoor Orientation Program to render medical treatment should it be required during this trip. In the event that a parent or guardian cannot be reached, I hereby give permission for transport to and treatment at a hospital facility. I have given up all future claims against Columbia University and its partner organizations. In addition, I understand that I am releasing Columbia University of all liability related to any aspect of my participation in the COOP Fall trip.
I certify that I have read this entire release, or that it has been read to me, and that I understand the legal responsibility I am assuming.
