Statement of Acceptance and Understanding

The following is a statement of acceptance and understanding of expectations to reside in a Special Interest Community at Columbia University. Please read this form carefully and check the box next to each statement, indicating that you have read and understand it. This form should be submitted by Monday, February 6, 2012 at 12 noon. Please save a copy of this form for your own records prior to submitting.

Please indicate whether or not you accept your placement offer.  If you accept placement, please read carefully and select the box next to each expectation statement to indicate that you agree to fulfill the expectation while residing in an SIC. Failure to fulfill Individual Membership Requirements may result in a student's termination of residence in the SIC suite and may jeopardize future eligibility for SIC membership or participation in the Room Selection process.
