On this page, you will find links to the required semester recogntion forms, to be submitted by the end of the first week of classes each semester. Please read all information carefully.

Chapter Semester Form and Roster

Please fill out the required semesterly chapter information, and upload the appropriate Chapter Roster to this form. Email submissions will not be accepted.
