CCSC and ESC Travel/Retreat Waiver

Please fill out this form completely in order to participate in the Student Councils' Retreat on Friday, September 28 and Saturday, September 29th to Frost Valley YMCA in Claryville, New York.

Confidentiality Notice: A digital copy of this form will be taken with the SDA advisors. Please note that this form contains confidential information and will be handled accordingly. The information contained in this form will be only used for emergencies only.

 

Medical Insurance: Students participating in overnight trips associated with a student organization are required to have adequate medical coverage. Please share the provider and policy number of the insurance plan you will be covered by during this period. It is the responsibility of the student to have proof of medical insurance on his/her person during the trip.
If a student is not 18 by the start of the trip, a hard copy of this form will need to be processed with a parent/guardian signature.
Please let us know of your dietary needs. If you observe Kosher or Halal dietary laws, please let us know if eating vegetarian meals are ok.

I am exercising my free choice to participate voluntarily in the Columbia Outdoor Orientation Program’s (COOP) Fall trip. I recognize that this decision involves certain risks and I am willing to accept responsibility for those risks.

Columbia Outdoor Orientation Program (COOP) Participant Consent Form

I understand that the goals of Columbia Outdoor Orientation Program (COOP) are to provide a positive experience in a supportive group environment. I am aware that as a participant I am required to follow the policies and guidelines set by COOP and instructions from the Outdoor Orientation Leaders. As part of Columbia University, COOP adheres to the University standards of respect for others. COOP is a substance-free program- alcohol or drugs of any kind are prohibited. Any violation of the substance policy may result in judiciary action.

Live at Lerner Volunteer Application

Thank you for your interest in being a part of Live at Lerner for the 2014 - 2015 academic year.  We are looking for a volunteer to help us plan events for Live at Lerner that will help bring the Columbia community together. If you are interested in being a part of Live at Lerner to help foster community in Lerner Hall and elsewhere at Columbia, consider applying to be a volunteer.
