Guest Attestation Form

Please complete this form just prior to your arrival on campus.

Each student is permitted one guest to assist them when they move into or check out of their residence hall. Any guest visiting the Columbia University residence halls for move-in or check-out must fill out the below form. Upon completion of this form, you will receive an email receipt. Please save this receipt so that you can present it to move-in staff.

**Important Notice: If you develop symptoms while on the premises, you must immediately leave the campus.**

Dates, Requirements and Protocols

Non-seniors check out by April 27 at noon
Graduating seniors check out by May 1 at noon

After showing this form, one guest arriving from within the United States will be permitted inside the residence hall with their student. Guests arriving from an international country may not enter the residence halls. All guests must comply fully with the terms of the Columbia Community Health Compact, including wearing masks at all times and adhering to physical distancing practices.

Check in April 29 and 30

Note that guests may not accompany students to the Check-In station or COVID-19 testing center in Lerner Hall.  Guests must wait for their student at their residence hall, where they will be asked to show the results of this Symptom Attestation Form. After showing this form, one guest arriving from within the United States will be permitted inside the residence hall with their student. Guests arriving from an international country may not enter the residence halls. All guests must comply fully with the terms of the Columbia Community Health Compact, including wearing masks at all times and adhering to physical distancing practices.

  • NOT experienced any symptoms of COVID-19 in the past 14 days (fever, cough, shortness of breath or difficulty breathing, fatigue, chills, headache, repeated shaking with chills, nausea, muscle pain, sore throat, abdominal pain/diarrhea, congestion or runny nose, or new loss of taste or smell);
  • NOT tested positive for COVID-19 in the past 14 days;
  • NOT knowingly have been in close or proximate contact in the past 14 days with anyone who has tested positive for COVID-19 or who has or had symptoms of COVID-19; and
  • (regardless of my citizenship status) fully complied with the vaccination, travel testing and quarantine policies of the Centers for Disease Control(CDC)
