If you need any help in assessing a project or initiative, collecting data, or just analyzing your current data, please submit the simple form below.

Domestic Travel Form

All students who travel domestically within the United States or its territories (outside of New York City) on Columbia-supported travel must register their trip with the University. To register a trip so it appears on this form, please contact your adviser.

Please complete this form at least two weeks before your departure. This form provides us with vital information in case of an emergency during your trip.

This form is for all Resident and or Community Advisors (RA/CAs) to submit after checking resident rooms upon check out. 

  • Please indicate each room and each uni for spaces which have room damage(s). 
  • If a room/suite has more than one resident, please make sure to note all residents and rooms.
  • You do not need to submit rooms and unis which do not have damages. 
  • If a photograph is warranted, please upload the photo to this webform.
