CUSP Summer Enhancement Fellowship Faculty Support Form

Dear colleague,

We hope that this message finds you, and all whom you know, safe and well.

The student sending to you this webform applied for the Columbia Undergraduate Scholars Program (CUSP) Summer Enhancement Independent Project Fellowship at Columbia University and has requested that you provide confirmation of your knowledge and support of their summer project proposal.  We offer you the ability to submit your endorsement online through our secure website. 

Please complete the short form below.  You will be requested to enter the student's name, University Network ID (UNI), and email address for verification purposes.  The form is intended to be completed in one sitting.  Once your support has been submitted, you will not be able to make changes.  The deadline for submission is Friday, May 22nd, 2020.
