On this page, you will find links to the required semester recogntion forms, to be submitted by the end of the first week of classes each semester. Please read all information carefully.

Semester Forms

The Chapter Information Form must be submitted by the first day of classes each semester. The Chapter Information Form requires the chapter roster be uploaded using the required template found below. Only submissions using the required template will be accepted- emailed chapter rosters will not be accepted.

Please complete the following forms by the appropriate deadline.

For the Anti Hazing Chapter and Anti Hazing New Member forms, submit hard copies to the Office of Residential Programs Fraternity & Sorority Life, 515 Lerner Hall.
